What is a Student Transition Placement Specialist?

The purpose of the position is to assist in preparing high school age adolescents for adult responsibilities through exposure to work readiness, life skills and self-advocacy training. In addition, this same group will participate in career exploration through informational and mock interviews, job shadowing and short term on the job experiences.

Role and Responsibilities:

  • Receive all referrals for the services provider under the DORS Level Up program.
  • Meet with DORS representative and identified school personnel to process referrals and schedule services.
  • Plan for, schedule services, and provide services to students within the approved locations.
  • Follow established curriculums and update as requested by supervisor of DORS.
  • Maintain effective communication with stakeholders on a regular basis.
  • Ensure that the necessary technology is available for student use.
  • Complete all necessary paperwork in a timely manner and submit, as needed, for service completion (including but not limited to intake paperwork, progress notes, final report, billing documents, and other paperwork required pre-service, during services, and post-service.)
  • Responsible to data entry related to services and program participant wages.
  • Identify community business partners in order to provide required services to the program participants.
  • Provide direct supports or collaborate with supervisor to identify additional staffing to work with program participants.
  • Must communicate effectively with the team in addition to networking with other business in order to provide placements for students to be able to participate in job shadowing/training.
  • Works with DAD and DDS referrals assisting individuals to navigate the community to find employment and volunteer opportunities.
  • Assist with on the job orientation and training, job coaching and providing intermittent support to the person and employer as needed

Essential Job Functions:

  • Ability to use technology and teach others to use technology for the purpose of providing services.
  • Must be able to organize self and the scheduling for student services
  • Must be able to follow directions
  • Must be able to document observational behaviors for reports.
  • Must be able to network and develop potential training/placement sites.
  • Ability to drive self to and from service locations.

Qualifications and Education Requirements:

  • College Degree preferred
  • Minimum of two (2) years preferred working with individuals with disabilities within the social system.
