Overview

The Advanced Practice provider (NP/PA) functions in collaboration with the Digestive Health Colorectal Screening Program and works towards developing and promoting colon cancer screening and surveillance in the community and throughout our YNHHS clinical and primary care practices. The APP coordinator will conduct outreach efforts in the community and physician practices, to improve awareness and enroll patients in the Digestive Health CRS Program according to YNNH and internationally approved clinical guidelines. They will develop and maintain a clinical database and registry. They will also coordinate the Digestive Health Pancreatic Cyst Surveillance Clinic and be involved with examining patients, including both new and follow-up patients in conjunction with the Director and allied members in Digestive Health. The APP Coordinator will also support patients in the clinic and working with the Director and allied members of the Digestive Health team. The APP Coordinator will provide oversight and management including the management of referrals. The APP will serve as a leading and integral member of a larger interdisciplinary team contributing to excellence and patient care, research and teaching. Clinical responsibility will include collaborating with other healthcare professionals throughout the continuum, performing clinical assessments as needed, concepts, principles, theories, and outcomes to solve complex problems, acquiring and managing pertinent patient data and tracking devices and related support. Provides exceptional direct diagnostic and therapeutic patient care to the highest standards of the organization as part of a multidisciplinary team. Cultivates a patient centric environment which focuses on the whole individual inclusive of physical, psychosocial, spiritual and functional needs of the patient, family, and significant others. Serves as the focal professional for the continuity of care in patient care within scope of practice standards, as well as specialty specific standards to address and resolve problems that may arise.

Responsibilities

  • 1. Clinical Care * The Advanced Practice Provider (NP/ PA) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team. The APP, whether the practice setting is the inpatient, ambulatory, or outpatient settings, provides optimal patient care, from diagnostic to prognostic matters. This individual works in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols and clinical pathways and may provide autonomous team based care. * Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic or pathology results. The APP may perform procedures, prescribes treatment and medications in accordance with institutional and state guidelines, as well as, when necessary for scope within specialty or professional competency standards. In some clinical specialties, the APP may have autonomous or independent practice managing certain patient populations. * Other clinical job duties may be applied in accordance with departmental and operational goals, training related to profession and practice necessitated guidelines and responsibilities, inclusive but not limited to responding to emergency situations within the YNHHS parameters
  • 2. Information Management * The APP ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure
  • 3. Quality Management * The APP is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations. The Program Coordinator will report on quality metrics to the steering committee
  • 4. Professional Development * The APP continuously engages in projects that maintain and advance professional competency. * The APP serves as a clinical resource for new house staff and may provide service specific orientation pertinent to clinical education * Consistently demonstrates high reliability organizational behaviors through reporting safety events as well as near misses. * The APP continuously engages in projects that maintain and advance professional competency. * Maintains all employee requirements determined by hospital policy * The APP may serve as a clinical preceptor for NP students and other health care professionals and may serve as a mentor to PA/ NP candidates. * The APP may collaborate on research, quality assurance projects, and assist with the formation of protocols that will be applied to clinical practice or further their educational and clinical expertise.

Qualifications

EDUCATION

Graduate from an accredited Nurse Practitioner Program or Physician Assistant Program

EXPERIENCE

Prefer clinical working experience as an NP or PA within the given specialty/service line recommended, but will consider a new graduate.

LICENSURE

Connecticut State Registered Nurse License Connecticut State Nurse Practitioner License or Physician Assistant State License Board specialty certification, as appropriate for position profile ACLS/BLS Certified, when indicated for position profile

May require travel for outreach in the community


Additional Information

This position requires a 40 hour per week schedule and will be M-F days with weekends and holidays as needed. The APP will function in collaboration with Director of GI Cancer Prevention Program and work towards increasing the overall colorectal cancer screening exams through outreach efforts in the community, physician practices and federal qualified healthcare centers to enlist patients and guide them through the algorithm for CRS and prevention. 1-2 years GI or cancer screening experience is preferred, but new grads will be considered. You will establish a clinical practice to riskstatify patients to increase access and throughput for our gastroenterology practice.
